Upper Lobe Cavitary Lesion Differential Diagnosis and Case Report

A patient who applied to our clinic with shoulder pain had a chest X-ray taken for differential diagnosis and a thorax CT scan was performed. A cavitary lesion was observed in the right upper lobe in the CT scan, and since the patient had a 25 p/y active smoker and only shoulder pain, no accompanying respiratory symptoms and normal infectious parameters in blood tests, malignancy was considered in the differential diagnosis. PET and biopsy procedures were planned for the patient in terms of malignancy examination. SUV max: 2.3 was found in PET-CT and biopsy was recommended. Since the patient developed shortness of breath until the biopsy procedure and the D-Dimer value checked for differential diagnosis was 4000 (lab upper limit: 500), CT angiography, pulmonary artery were performed. Anticoagulant treatment was administered to the patient after the detection of embolism in the CT angiography and a significant regression was observed in the lesion in the right upper lobe of the lung in the control CT scan 1 month later.
