Scapular Bronchogenic Cyst: Systematic Review of Clinical, Diagnostic, and Therapeutic Aspects

Background: Scapular bronchogenic cysts are rare congenital malformations originating from ectopic budding of the tracheobronchial tree during embryogenesis. This study aims to understand the epidemiological, clinical, diagnostic, and histopathological characteristics of this malformation.

Methods: A systematic review of 35 cases described in 33 studies published in scientific literature between 1971 and 2022. Publications in English, Spanish, and French were included; case reports, case series, or letters to the editor describing patients diagnosed with scapular bronchogenic cyst and with complete information on diagnostic tests, therapeutic approach, and histopathological findings.

Results: There was a predominance of cases in children (median age of 3 years) and males (74.29%), with a preferential location on the left side of the scapula (57.14%). Most patients did not undergo preoperative imaging tests (57.14%), and in those who did, the clinical diagnosis was not established. Surgical treatment was universally adopted, with histopathological confirmation in 94,28% of cases, showing pseudostratified columnar epithelium, smooth muscle in 51%, cartilage in 5.71%, as well as stratified squamous epithelium and mucous glands.

Conclusion: Despite the rarity of scapular bronchogenic cyst, this review highlights the importance of awareness of this condition to improve early diagnosis and optimize clinical management. Future studies should investigate the embryological mechanisms and standardize diagnostic approaches.
