Hydatidosis, a parasitic disease arising from cosmopolitan anthropozoonosis, affects humans as accidental intermediate hosts. The surgical incidence is 4.55 cases per 100,000 inhabitants, with a prevalence in Morocco of 7.76 per 100,000 inhabitants, less prevalent in children compared to adults. While intrabronchial rupture is common in hydatid disease, intrapleural rupture is rare, with reported frequencies ranging from 1.5% to 6%, even in endemic regions. A simultaneous rupture at the bronchial and pleural levels is even rarer and more severe. We present the case of an 8-year-old child, undergoing treatment for a hepatic hydatid cyst, who experienced vomiting and chest pain. We admitted the patient to manage an intrapleural hydatid cyst, which we identified following abdominal pain and hydatid vomiting.
