Presentation of an incidental case of gastric perforation in a twin pregnancy that was interned with the indication of threatened preterm labor (TPTL). A 33-year-old patient with gravida 2 and parity 1 presented to our emergency department with the complaint of abdominal pain. The pregnant woman was interned to the delivery room. During the follow-up period with TPTL protocol, the patient's general condition deteriorated and she went into acute renal failure thus urgent C/S decision was taken. Two stillborn babies were delivered. During the operation, the patient was consulted to General Surgery perop due to the intra-abdominal diffuse purulent material with dark content thought to be gastrointestinal tract contents. The patient was evaluated as gastric perforation by general surgery and Graham Rafi repair was performed. The patient was followed up for 12 days in intensive care unit on level three for close postoperative follow-up and observation. Afterwards, the patient was followed up in the General Surgery clinic and discharged on post-op 32nd day with recommendations.
