Endodontic therapies remain a difficult and demanding discipline, which cannot forgive approximation.

Part of the failures is linked to the non-respect of the recommendations of good practice, concerning the respect of the asepsis rules and a good radiography interpretation, leading to an underestimation or a lack of information concerning the complexity and the difficulties of the case.

The improvement of initial training, with the clinical teaching of simplified but scientifically based techniques, and the continuous training should improve these standards and enhance the success rate of endodontic treatments. At the same time, the technological advances of the last two decades have made it possible to push the technical limits of these procedures and to extend the indications to handle clinical situations that were unmanageable a few years ago.

The aim of these case studies is to present the different endodontic access difficulties encountered before and during root canal treatment and the management of these complexities by starting with good fundamental examination including the patient’s history, clinical results, and radiographic examinations.
