Eagle syndrome is rare clinical disorder that occurs when an elongated styloid process presents with or without calcification of the stylohyoid ligament. Symptomatic cases may present with neurological symptoms and continuous or intermittent pain in the face and/or neck region(s). In this symptomatic patient, there was radiographic evidence of greater than 90% narrowing in bilateral internal jugular veins below the skull base and between the C1 transverse process and styloid process. Using intraoperative neurophysiological monitoring ensured adequate cerebral blood flow under general anesthesia in the lateral decubitus position.
